Pre admission

Once the decision has been made that you require admission to Dudley Private Hospital, we ask that you deliver, post or fax your admission kit to the hospital as soon as possible (please ensure that original is brought with you on admission).

Your admission kit, which you should have received from your doctor, will contain the following forms, all which need to be forwarded to the hospital prior to admission:

• Doctors Referral Form (to be completed by your doctor)
• Pre-Admission Registration Form (to be complete in full by the patient or guardian)
• Patient History Form (to be complete in full by the patient or guardian)

Our reception is open Monday to Friday, 7am to 5pm. It is preferable that you attend in person to deliver the necessary paperwork, but if this is not possible then you can post or fax your admission papers.

The day before your admission
Please call our Pre-Admission office the afternoon prior to your admission between 1pm and 4pm to obtain your admission and fasting details.

Depending on the type of surgery you are having and any special requests by your surgeon or anaesthetist; you may be contacted by our Pre Admission Nurse.

What to bring
On the day of admission, please bring:

• Medicare card
• Health Fund Details (if applicable)
• Pension and/or Veterans’ Affairs details (if applicable)
• Workers’ Compensation or Third Party Claim details (if applicable)
• Pharmaceutical Safety Net Card and/or Health Care Card (if applicable)
• X-rays and pathology test results requested by your doctor
• Personal toiletries
• Medications you are currently taking in their original containers, clearly labelled and any repeat prescriptions. A current Personal Medication List would also be useful. All medications required during your stay will be administered by the nursing staff.

Please do not bring large amounts of clothing, baggage or valuables.

Accommodation
At Dudley Private Hospital our aim is to make your stay as pleasant and comfortable as possible. Featuring both shared and single rooms, a 4 bed paediatric ward and 4 bed High Dependency Room, we attempt to accommodate the special needs of all our patients. All rooms have external views and feature private bathrooms, television and telephone.

While we will make every endeavour to provide you with your accommodation preference, please understand that in certain circumstances of high activity or emergency admissions, it may be necessary to offer you a shared room until a private room becomes available.
